Documents required to get the death certificate
To obtain birth certificate in Azerbaijan you need to submit the following documents to the nearest ASAN Service center or to the district register office of Civil acts (ZAQS) of the Ministry of Justice of Azerbaijan:
1) Reference on death fact (Form 106);
2) Court decision on recognizing the person deceased or establishing the fact of death;
3) Deceased person’s ID card (if a person was a foreigner and permanently lived in Azerbaijan, then his Permanent Residency Card; if person was under 18, then his birth certificate);
4) Military ID (if deceased was liable for military service and conscript).
