So, let’s discuss the procedure step by step. First of all, let’s start from legal basis.
Legal basis, that covers this process is Civil Code of Azerbaijan, as well as Rules for changing name, surname and patronymic, approved by Cabinet of Ministers of Azerbaijan.
Reaching 18 years, any person can apply to change his name, surname and patronymic in Azerbaijan.
Where to apply to change name in Azerbaijan?
To change your name, surname and patronymic you need to submit application form and the documents specified below to the nearest ASAN Service, district department of Registration of Civil Acts or consulate of Azerbaijan in foreign countries.
Application should contain the following information:
– Current and desired name, surname, patronymic;
– Date and place of birth;
– Nationality and citizenship;
– Place of registration, place of work and position;
– Place of military registration;
– Info about changement of name, surname and patronymic;
– Info about marriage (incl. divorse) and children;
– Deportation from foreign countries (indicating the exact country and date);
– ID (series, number and date of issue).
Documents required to change name and surname in Azerbaijan
In addition to the application on changing the name, surname and patronymic you also need to submit the following documents:
1) Birth certificate;
2) Marriage certificate (if applicant is married);
3) Divorce certificate (if applicant is divorced);
4) Birth certificate of minor children (if any);
5) ID card (notary signed copy);
6) Registration card (on initial military registration) or military ID;
7) 2 photo sized 3х4 cm;
8) Document confirming payment of the state duty.

In which period can I obtain certificate on changing name, surname and patronymic?
District department of Registration of Civil Acts, ASAN Service or consulate of Azerbaijan in foreign countries usually issues certificate in 30-40 days. In case of specified authorities decide that changing name, surname or patronymic is impossible, they issue negative decision.
What is the duty for changin name, surname and patronymic in Azerbaijan?
Currently the state duty is 15 AZN.
Can I take surname of my upbringer?
By agreement with the upbringer and at the request of the applicant, it is possible to take the upbringer’s surname.
Can I change ending of my surname?
Yes, changing of surname’s ending is carried out on the same basis, as indicated in this article.
In case of changing of surname’s ending of children who are under 10 years old, their consent is required.
Can I change the ending of my surname once again?
Changing ending of a surname is only possible once until person becomes 18 years old.
